Change Group Request Form

1) You are allowed to change the group if it clash with other subject or group. You are only
allowed to change group ONCE.

2) If there is no clash, you are allowed to change group if you could get someone who willing to change from the opposite group.

3) The changes are subject to management approval and the process shall take 5 working days.

4) One form is applicable for one group change only. If you are intending to change 2 subject grouping, then you need to complete 2 forms.
