1) This form need to be submitted within 24 hours upon your absence from class or exam.
2) You are required to submit the hardcopy of MC complete with the signature of lecturer and details within 24 hours from the date of MC.
3) Upon completing this form, kindly forward the email that you shall receive with attachment of the proof either from Government Medical Certificate, ticket, any certificate to rozzaelena.amc@gmail.com within 24 hours upon your absence from class or exam.
4) One form is applicable for one subject exemption only. If you are absence for 2 subjects, then you need to complete 2 forms.
