Transfer Credit Request Form

1) To apply for credit transfer, you must first completed minimum diploma which has been formally assessed with the certificate and transcript officially awarded.

2) You are required to provide the full syllabus and course outline according to MQA format along with your certificate and transcript on the intended credit transfer course/s. The course need to be graded with minimum C (50%) and 80% map with the syllabus in UniCAM.

3) The fee charge is RM 100.00 per subject payable upon request. 50% of the fee will be refunded if your transfer credit request is not approved.

4) One form is applicable for one course only. If you are intending to transfer 3 courses, then you need to complete 3 forms.
